A Chronic Disease Management Plan is a specialised treatment plan for individuals with long-term health conditions. This involves assessing your medical history, functional capacity, and goals to deliver a safe individualised treatment program. The treatment program focuses on implementing exercise prescription tailored to your condition and lifestyle interventions to best support your health and well-being. The long term goal is to successfully manage symptoms and adopt strategies to allow for sustainable self management.
This service is designed to support your recovery from injury through specialised, evidence-based exercise programming. Optimising your rehabilitation by taking into consideration your limitations, required daily activities, occupational requirements and sporting goals. Each treatment plan is tailored to your musculoskeletal injuries and focuses on restoring range of motion, function, mobility and strength. Progression will focus on biomechanical corrections and increasing pain free ranges of motion to allow a return to all desired activities.
Advanced exercise prescription involves delivery of highly individualised, evidence-based programs tailored to complex needs. This incorporates chronic conditions, post-injury rehabilitation, occupational rehabilitation and performance goals. Exercise is prescribed with precise control over mode, intensity, volume, and duration to ensure treatment is safe and effective. Treatment may involve strength training, cardiovascular conditioning, functional movement strategies and/or mobility programs.
